The networks below offer health care providers and stakeholders an opportunity to collaborate to accomplish better care, better health and more affordable care through quality improvement. Click on a network name to learn more about what it offers, or view a full list of our network resources.

The Networks section of the site is available to members only.  If you do not have an account, you may create a new account. If you are a registered user, please log in with your existing account and then join the networks below.

Benefits of participation include connecting and collaborating with others in your field and sharing knowledge through forums, listservs and educational events. You will have access to tools and resources specific to each network, as well as easy event registration

If you are a provider working in Puerto Rico and would prefer to view content in Spanish, when available, create an account, if you have not already done so, or log in with your existing account. Once you are logged in you may select the Puerto Rican flag at the top of the website to access Spanish content. The next time you log in you will automatically get the content in the language you’ve chosen.

The Nursing Home Quality Improvement Network works to improve quality of care delivery to nursing home residents.

Patient and Family

This network provides Medicare patients and their families valuable information and the latest news on The Centers for Medicaid & Medicare (CMS) quality improvement projects. In addition, there are tools and resources to help you improve your health and experience with the health care system.

Quality Improvement Initiative

The Quality Improvement Initiative Network collaborates with practitioners and providers to improve the quality of health care provided.

Quality Improvement through the Merit-based Incentive Payment System and Advanced Alternative Payment Models.


The Readmissions Network fosters community coalitions of patients, caregivers and clinical teams to improve the quality of patients’ transitions between health care settings and reduce avoidable hospital readmissions.
