"Taking Care of Myself: A Guide for When I Leave the Hospital" helps prepare patients for self-care and follow-up before leaving the hospital. Available in English and Spanish as a printed document or fillable PDF, this guide was adapted from Project RED (Re-Engineered Discharge).
